Infuse Land offers grant writing, technical assistance and grants administration for rural schools, growers, nonprofits and community partners working on food, farm and health projects.
We know grant applications can feel like a lot, especially for small teams already doing important work. Our role is simple: to listen, to help you put your project into clear words and complete paperwork, and to stay alongside you after the award so the details are handled with care.
We believe rural communities deserve steady, thoughtful support. We work with partner-approved data governance and community review, so the people closest to a project help shape how its story is told.
Listening & planning. We start by learning about your project, your partners and your goals, then look together for funding that fits.
Application writing. Clear, complete narratives that describe your work honestly and are ready for review.
Budgets & work plans. Complete, accurate work plans and budgets that meet program requirements and are ready to submit.
Readiness & compliance. Gentle guidance on registrations, eligibility paperwork and federal grant requirements.
Post-award administration. Help with expense tracking, reimbursement requests and progress reporting, so you can stay focused on your community.
The Rural Health Transformation Program is a federal program run by the Centers for Medicare & Medicaid Services (CMS). It was created by Section 71401 of Public Law 119-21. CMS funds states, and each state decides how to invest in its own rural communities. Many states offer subawards to local organizations.
CMS describes the program's goals as helping rural communities by:
promoting prevention and addressing root causes of disease,
helping rural providers stay sustainable, long-term places to get care,
strengthening the rural health workforce,
encouraging new models of care, and
supporting technology that brings care closer to home.
Every state runs its own application process, so details vary. We're glad to help you:
understand your state's opportunity and whether your project is a good fit,
prepare a clear, complete application, and
keep up with reporting and reimbursement after an award.

Whether you're a school, a local farmer, a food hub or a community organization, we'd be glad to hear what you're working on.
Schools & food service teams: fresh food, kitchens and growing spaces for students.
Growers & food hubs: infrastructure that helps local food reach local tables.
Nonprofits & community groups: storage, delivery and food access in your area.
There's no pressure. A short conversation is a fine place to start.
